Bausch + Lomb will review applications for Independent Research Grants, Educational Grants and Charitable Contributions made by persons or organizations in the United States in accordance with FDA Guidance regarding Industry-Supported Scientific and Educational Activities, the Office of Inspector General Compliance Program Guidance, The Accreditation Counsel for Continuing Medical Education Guidelines, the PhRMA Code on Interactions with Healthcare Professionals, the AdvaMed Code of Ethics on Interactions with Healthcare Providers and internal Bausch & Lomb policies. Applications received from persons or organizations outside of the United States will be reviewed in accordance with applicable laws and ethical guidelines as well as internal Bausch + Lomb policies.
Minimum Requirements
The following are the Bausch + Lomb minimum requirements that applicants for Grants and Charitable Contributions must satisfy. Please note that applicants will be competing for limited funds set aside for Grants and Charitable Contributions. Satisfying the minimum requirements specified herein is no guarantee that an applicant’s proposal will be approved for funding.
General Minimum Requirements Applicable to all Grants and Charitable Contributions
Under no circumstances will Bausch + Lomb provide funding for an Educational Grant, Independent Research Grant or Charitable Contribution that is related to or otherwise conditioned upon the past, present or future prescribing, purchasing or recommending of Bausch + Lomb products. No consideration will be given to the applicant’s past, present or future commercial relationship with Bausch + Lomb. Any communication suggesting that a Grant or a Charitable Contribution is conditioned on the applicant’s current or future commercial relationship with Bausch + Lomb will result in the disapproval of the associated application.
Educational Grants

The educational event must be primarily dedicated to promoting objective scientific and educational activities and discourse.

The educational event sponsor must select the attending healthcare professionals and be responsible for and controls the selection of program content, faculty, educational methods, and materials.

The educational event will be conducted in locations that are conducive to the exchange of information.

The educational event is non-promotional and free from commercial influence.

Bausch + Lomb will be acknowledged by the grant recipient as a funding source at the educational event.

Educational grant funding will not be used to pay for the applicant’s ordinary operating expenses.

Educational grant funding will not be used to pay for the travel, lodging or other personal expenses of non-faculty healthcare providers, except for medical students, fellows, residents, or other healthcare professionals in training selected by an educational institution or a conference sponsor, except as may be permitted by law and Company policy in countries other than the United States.

Time spent by non-faculty healthcare professionals attending the conference or meeting or for promotional exhibits, or for lavish venues, meals or other entertainment.

The applicant is not requesting funds for or on behalf of a government official, politician, government organization or political party.

The applicant has no criminal record and is not excluded from participation in government healthcare programs.

The applicant has not refused or failed to satisfy all accounting and/or contractual requirements associated with any prior Bausch + Lomb Educational Grants they may have received.

The applicant has not refused or failed to satisfy all accounting and/or contractual requirements associated with any prior Bausch + Lomb Educational Grants they may have received.

The applicant is an organization qualified to engage in and/or fund the proposed educational activities such as an academic institution, medical or professional association, medical education company, community health center, conference organizer, patient association or advocacy group.

The applicant certifies and provides documentation evidencing that the educational program for which they seek funding is accredited by ACCME, ACPE, ANCC, EACCME, COPE, JCAHO, AMA or some other relevant independent educational standards organization, or that they are otherwise qualified to perform the proposed educational activity.

The applicant agrees to sign Bausch + Lomb’s Educational Grant Agreement.

Independent Research Grant

Applicants must be seeking funding for research related to eye health or a technological development that is designed to provide valuable scientific/clinical information relevant to disease states, drug or device efficacy, device design, improved clinical care, new clinical use or otherwise benefits patients that is consistent with the Company’s stated research goals established by one or more Independent Research Grant Team.

Applicants must have complete control over all aspects of the proposed research.

Applicants will perform the funded research in full compliance with all applicable laws and ethical standards.

The Independent Research Grant must not be used to fund an applicant’s ordinary operating expenses unrelated to the funded research project.

Applicant is not requesting funds for or on behalf of a government official, politician, government organization or political party

Applicant does not have a criminal record and is not excluded from participation in government healthcare programs.

Applicant has not refused or failed to satisfy all accounting and/or contractual requirements associated with any prior Bausch + Lomb Independent Research Grants they may have received.

Applicant is a bona fide researcher qualified to perform the proposed research in a timely, competent and lawful manner.

Applicant agrees to sign Bausch + Lomb’s Independent Research Grant Agreement prior to funding.

The proposed research protocol satisfies relevant legal requirements and industry standards.

The applicant submits a complete application, along with all associated documentation in sufficient time prior to the commencement of the proposed research project to provide Bausch + Lomb the time to adequately review and act upon the application.

The amount of the Independent Research Grant requested is reasonable in light of the activities to be performed.

Charitable Contributions

The application seeks funding in support of public or patient education particularly in support of eye health, community betterment, or other similar charitable purposes.

The application seeks sponsorship of events where the proceeds are intended for charitable purposes.

Bausch + Lomb will not provide Charitable Contributions for:

Raffles, courtesy advertising, golf outings, trips, books, documentaries and videos (unless the books or videos are eye health related).

Endowment funds or funds directed at deficit reductions or operating reserves.

Annual operating support, with the exception of selected community institutions that the Bausch + Lomb currently funds.

2 Applicant is not requesting funds for or on behalf of a government official, politician, government organization or political party.

3 Applicant is not a labor or fraternal organization or entity.

4 Applicant is not an amateur or professional sporting organization.

5 Applicant does not have a criminal record and is not excluded from participation in government healthcare programs.

6 Applicant has fully satisfied all accounting and/or contractual requirements associated with any prior Bausch + Lomb Charitable Contributions.
